Guide me to get SWORN Affidavit from Chicago Indian embassy

I would like to apply MINOR passport for my 
daughter who is staying in India. To apply 
passport, I need to get SWORN Affidavit from Indian 
embassy. Can someone guide me.

Do I need to go in person to get the affidavit or 
can I apply through mail?
What are the documents required?
Do we have form to fill?
Any processing fee?
Do you have any contact number to reach the 
consulate members directly.
